Procedure on School ID Application

New Students(Freshmen, Transferees and Cross Enrollees)
  • Present Computerized Print-out to ID Personnel.
  • Claim ID @ Dean’s Office

Old Students
  • Proceed to ID Validation
  • Present “ANY OLD ID” for Validating Sticker placement.

Old Students
Who lost their ID’s
  • Pay Php 70.00 at the Cashier.
  • Get and Accomplish ID Replacement Form from the Office of the Student Affairs.
  • Present Receipt, ID Replacement Form & Print-out to ID Personnel
  • Claim ID @ Dean’s Office
